Every non-immigrant visa applicant must have a completed and signed DS-156 form. For persons under the age of 16, the form may be completed and signed by one of the parents or a legal guardian. All three pages of the DS-156 must be filled out online and printed.

The following supplemental form DS-157 must be completed by all male non-immigrant visa applicants between the ages of 16-45.

The following supplemental form DS-158 must be completed by all F, J, and M visa applicants, including spouses and children applying for F2, J2, and M2 visas.
